UNDERSTANDING YOUR SYMPTOMS
Understanding back and neck pain
Back and neck pain can have several contributing factors, and similar symptoms may arise for different reasons in different people.
Joints, muscles, nerves, activity load, sleep, work demands, previous injury and sensitivity may all be relevant. Pain is real even when imaging does not show one clear cause, and common scan findings do not always match the severity or impact of symptoms.
Assessment therefore avoids assuming that every episode is caused by a slipped disc or one structural problem. It considers your history, function and relevant physical findings, while recognising when symptoms need medical investigation rather than physiotherapy alone.
COMMON PRESENTATIONS
Symptoms and situations we commonly assess
These examples can be explored in physiotherapy, but they are not diagnoses and suitability depends on your individual assessment.
ASSESSMENT AND REHABILITATION
How physiotherapy may help
Your physiotherapist begins with the history of symptoms, health information, work and activity demands, and the movements that have become difficult. The physical assessment is selected according to your presentation and comfort.
Findings may include movement, strength and, when relevant, a neurological screen of sensation, reflexes or muscle function. They are explained in context rather than used to claim one certain cause for every symptom.
Understanding your symptoms and history
The assessment connects symptom behaviour with health, activity, sleep, work and previous episodes.
Movement, strength and neurological assessment
Relevant physical tests help explore function and identify whether further medical assessment may be appropriate.
Personalised exercise and activity progression
Movement and strengthening begin from a manageable level and progress towards daily or work goals.
Flare-up and self-management planning
Practical strategies can help you respond to symptom changes without assuming every flare-up means new damage.
The plan may include advice, exercise and hands-on treatment where appropriate, with progress reviewed through movement, confidence and meaningful daily activity.

FREQUENTLY ASKED QUESTIONS
Back and neck pain questions
Practical information before arranging an assessment.
What commonly causes back or neck pain?
Symptoms can involve joints, muscles, nerves, activity load, work demands, sleep, sensitivity and previous injury. Often several factors contribute, so assessment is more useful than assuming one cause from the location of pain alone.
Do I need a scan before seeing a physiotherapist?
Usually not. Many people can begin with clinical assessment, and common scan findings do not always match symptoms. Imaging may be recommended by an appropriate clinician when the history or examination suggests it would change management.
Can physiotherapy help pain that travels into my arm or leg?
Physiotherapy may help when spreading symptoms are suitable for assessment and rehabilitation. Because arm or leg symptoms can have different causes, a neurological screen and clear safety advice may form part of the appointment.
Is movement safe when my back or neck hurts?
Movement is often introduced gradually rather than avoided completely, but the appropriate level depends on your symptoms and health circumstances. Your physiotherapist can help identify a manageable starting point and adapt it according to response.
How many physiotherapy sessions might I need?
There is no standard number. Frequency depends on symptom duration, function, goals and response to rehabilitation. Reviews help decide whether appointments should continue, become less frequent or change focus.
When should back or neck pain receive urgent medical assessment?
Seek urgent advice for new or progressing major weakness, changes in bladder or bowel control, numbness around the saddle area, serious trauma or concerning systemic symptoms. NHS 111, your GP or emergency services can advise according to urgency.
